Settlement Information Last Updated on December 10, 2007:

The Court has granted final approval of the settlement.

Settlement benefits will be distributed after any appeals are resolved. The parties cannot predict whether any appeals will be filed and if so, how long they will take to resolve. In the event no appeals are filed, the parties expect that the settlement administrator will begin distributing settlement benefits in December 2007.

To receive benefits, you must complete and return a Claim Form. The deadline for mailing your Claim Form or submitting your Claim Form online is December 14, 2007.

Your calls will be answered Monday through Thursday from 8 a.m. to 8 p.m. Central time. On Friday, December 14, 2007, the call center will be open from 8 a.m. to midnight Central time.

Questions may also be submitted by email to claimsadmin@iowamicrosoftcase.com. Please include name, address, phone number and if available, the number under the barcode on your claim form along with your questions.

Claim Forms and Instructions.

To receive settlement benefits for your eligible purchases of Microsoft Windows, MS-DOS, Windows NT Workstation, Office, Excel, Word, Works Suite or Home Essentials 97 or 98, you will need to complete one of the two claim forms described below.

Please select either the Standard Claim Form or a Volume License Claim Form, whichever applies to you.

Standard Claim Form:

Use this form if you purchased a computer with Microsoft software already installed or if you purchased eligible Microsoft products at a retail store, by mail, or on the Internet between May 18,1994 and June 30, 2006, for use in Iowa, and not for resale. Do not use this form if you were an Open, Select or Enterprise volume licensing customer any time from May 18, 1994 through and including June 30, 2006 (see below). If you are completing a Claim Form on behalf of an Iowa state or local government, you must use a State and Local Government Claim Form or a Volume License Claim Form.

You may file your claim online if you are not a volume licensee and/or an Iowa state or local government and your claim does not exceed $200. You do not need to provide any additional documents or proof about your software for your initial $100 claim. You may file online for up to another $100 in claims (for a total claim not to exceed $200) if you are able to provide the Product Identification Numbers for the Microsoft products over the initial claim of $100.

 
Volume License Claim Form:

Use this form if you purchased any eligible Microsoft products between May 18,1994 and June 30, 2006 (from July 1, 2002 through and including June 30, 2006 for Iowa state and local governments), for use in Iowa, and not for resale, through Microsoft's Open, Select or Enterprise licensing plans. If you were a Volume License customer at any time from May 18, 1994 through and including June 30, 2006 (from July 1, 2002 through and including June 30, 2006 for Iowa state and local governments), you may make claims for all of your eligible Microsoft product purchases by using this claim form. You do not need to use the Standard Claim Form or a State and Local Government Claim Form.

State and Local Government Claim Form:

Use this form if you are an Iowa state or local government and purchased a computer with Microsoft software already installed or if you purchased eligible Microsoft products at a retail store, by mail, or on the Internet between July 1, 2002 through and including June 30, 2006, for use in Iowa, and not for resale. Do not use this form if you were an Open, Select or Enterprise volume licensing customer any time from July 1, 2002 through and including June 30, 2006 (see above).
